Appeals
- Satisfactory Academic Progress (SAP) Appeal
- Use this form if you’ve lost financial aid eligibility due to GPA, completion rate, or maximum timeframe issues. You’ll need to explain circumstances that have resulted in you needing an excpetion to the SAP policy.
Loans
- 2025-2026 Loan Form
- Use this form to request an increase, reinstatement, or redistribution of your Federal Direct Loans
- 2025-2026 Loan Cancellation/Reduction Form
- Submit this form to cancel or reduce all or part of your awarded financial aid (loans).
- Federal Direct Parent PLUS Loan Application
- Parents of dependent students use this form to apply for the Federal Direct Parent PLUS Loan. A credit check and PLUS MPN are required.

- Allows parents to authorize that any PLUS Loan refund be released directly to the student rather than to the parent borrower.
- 2025-2026 Private/Alternative Loan Self-Certification
- Required for students applying for a private or alternative education loan. Use this form to certify your previosly approved loan.

- 2025-2026 Professional Judgment Request

Use this form to request a financial aid review based on special or unusual circumstances.

Special circumstances include significant changes to your household income due to events like job loss, divorce, or the death of a parent or spouse.

Unusual circumstances apply if you are a dependent student but are unable to provide parent information on the FAFSA due to conditions such as abandonment, abuse, or estrangement.
Supporting documentation is required for all requests.
