Eligibility
All full-time employees are eligible to enroll in The Hartford insurance options. An employee's spouse and dependent child(ren) are also eligible to be enrolled under the employee's policy per the details of each applicable Policy Certificate.
What is Critical Illness Insurance?
Critical Illness Insurance provides a lump sum benefit if the employee or their covered dependents are diagnosed with a serious medical condition such as cancer, heart attack, or stroke. A benefit amount is also payable if a previously covered illness returns or if an additional covered illness is diagnosed while insured under the policy. Covered employees can use the payment in any way they choose, such as for expenses not covered by medical insurance or day-to-day living expenses.

What is Accident Insurance?
Accident Insurance provides a lump sum benefit if the employee or their covered dependents experience an accidental injury, related services, and treatments. Covered employees can use the payment in any way they choose, such as for expenses not covered by medical insurance or day-to-day living expenses.

Critical Illness Health Screening Benefit Form
If you are enrolled in The Hartford Critical Illness Insurance, you can receive a health screening benefit of $50 per covered individual per year for completing a preventative screening during the year. Examples of preventative health screenings are pap smears, fasting blood glucose test, colonoscopy, etc.
Each covered individual can complete the Health Screening Form and submit documentation of the office visit or lab work to serve as evidence of the health screening. A more comprehensive list of the health screenings are on the form. The claim form and documentation can be mailed or faxed to The Hartford according to the instructions at the top of the form.
