Event Intake Form

The purpose of this form is to improve communication among internal Lansing Community College Departments and Administration in the areas of planning, implementation, and evaluation of events.

Please complete this form at least two months prior to your scheduled event or activity and email any additional details (for example: RSVP info, attendee fee, maximum capacity, instructions, notes, catering needs, attachments/examples, etc.) with your submission. Once submitted, you will receive an email within seven days. Some activities may require additional information prior to approval.

Directions for Filling out the Event Intake Form:

  1. Before you begin your event, please thoroughly review this guidebook for event planning at Lansing Community College.
  2. Save the Event Intake Form to your computer and (if prompted) select "Enable Editing" at the top of the page.
  3. Completely fill out the form. It's important you be as detailed as possible, as these are questions the Event & Communications Coordinator will need to know to assist in coordinating the event.
  4. Make sure your completed document is saved to your computer.
  5. Email the completed document to with any additional details or attachments you think may be helpful.
  6. If you have not been contacted within 7 days of submitting your Event Intake Form, please reach out to the Event & Communications Coordinator to make sure they received your submission.
