Handbook
Students in the High School Diploma Completion Initiative must read and sign a document stating they have read and agree to the term as set fourth in the attached HSDCI Student Handbook.

Attendance Appeal Form
Please fill out the attached document for absences from class.

Team Time Exemption Form
Team Time is scheduled each semester from 9 am - 10 am, Monday through Friday.  If you are scheduled for a class that conflict with the Team Time requirements please have your instructor confirm you weekly attendance in class.

Daily Attendance Confirmation Form
Please have your instructor sign this form to confirm your attendance in your classes.
