How to Generate Assessment
Center Cover Sheets
Step 1: Log into Star
Port using your TUID
Step 2: Click on the My Place
tab. In the box entitled Work Links, click on
General Request Forms. This will take you to the
Test Referral Cover Sheets.
Section
1: Instructor, Course, and Student information
Step 3: On the request form, select whether the test is
Make up (face-to-face), Virtual, Hybrid
or Telecourse. (This releases the course list
according to the course type. If this is left unchecked,
you will not see your course to choose from.)
Step 4: Your name, e-mail, and phone number
should already appear in their respective fields. If
not, please enter them now.
Step 5: Under Term, select the term for which
the test is required from the pull down menu. You may
have to wait for the page to refresh, and this may take
a few moments.
Step 6:
Under Location, the Assessment Center will be the
default.
Step 7: Under Course, select the course name and
CRN from the pull down menu. You may have to wait for
the page to refresh, and this may take a few moments.
(In order to use the class list function, you must have
already selected a term.)
Step 8: Under Students, you can identify the
student(s) you need a cover sheet for by one of the
following methods:
o
To identify a single student, use the
small up and down arrows to locate and then
highlight,
by mouse clicking once, the name of the student.
o
To identify several non-contiguous
students in the list, hold down the Ctrl key and
click the name of each student you need a form for.
o
To identify the entire class, click on the
first name in the list, scroll down to the last name on
the list, then hold down the Shift key and click
on the last name. This will highlight all the names on
the list.
(Be
sure to confirm before you print that you have selected
all the students you need a cover sheet for.)
Step 9: Under Test ID, give the test a name or
enter a description. Use a simple name for your test,
for example: Test 5 or Midterm. (AVOID
using any special characters such as &,*,?,%,@,?, as
these may cause some problems.)
Step 10: Under Time Limit, set a time limit for
taking the test. If you choose to give a time limit, you
must do so using time markings (example: 2 hours or 2.5
hours). If you choose not to give a time limit, please
type none.
Step 11: Under Completion Date, enter the last
date that the student is allowed to take the test (you
may use the calendar link).
Step 12:
Click the Continue button at the top right of
the screen.
(Please
note: The Blank Form button at the top left of
the screen, allows for the printing of a blank form
should you want to complete one or more cover sheets by hand.
This button takes you directly to Step 16 below.)
Section
2: Materials and Special Instructions
Step 13:
Select the materials that you wish to be used with the
test.
o
No Materials-The student will not
be allowed to use any materials during the test.
o
Other Materials Allowed-The student
will be allowed only the materials that you have checked
off.
Step 14: Under Special Instructions, you may
type in any other specific instructions for the
student(s) taking the test. (AVOID using any special
characters such as &,*,?,%,@,?, as these may cause some
problems.)
Step 15:
Click on the Generate Forms button at the top
right of the screen.
Step 16:
After the cover sheet(s) have been generated, you have the
following options:
o
To Print: Click on the View
button. Once the window opens with the document
preview, go to File and select Print.
When printing more than one cover sheet, you may need to set
the margins to accommodate the particular printer you
are using.

To print to the printer in the CTE, set
the bottom margin to .955 to ensure each sheet prints on
its own page.
ü
To print to other printers you may need to
adjust your margins similarly through trial and error to
ensure each sheet prints on its own page.
o
To E-mail: At this time the
Assessment Center is not accepting Cover Sheets via
email. However, your department may support this option.
Contact your department to verify.

To Begin: This will take you back
to Section 1 at the beginning.
